ELIGIBILITY
REQUIREMENTS
Children
applying to Head Start must meet the following requirements:
Foster
children and families receiving Supplemental
Security Income, DPW
Cash Assistance, or DPW Transportation or Work Expenses are automatically eligible.
10%
of children are accepted that exceed the income guidelines. All families
are encouraged to apply.
For a child's application to be considered for
acceptance parent/guardian must provide:
Once a child is accepted, parents/guardians must complete the following to
complete the enrollment process:

Child's
Current Immunization
Record
-
Child's
Medical Insurance Card
-
Completed Physical Examination Form - must be completed & signed by the
child's physician within 90 days of enrollment.

Completed Dental Examination Form - must
be completed & signed by the child's dentist within 90 days of enrollment.

Statement
of Required Screenings completed by parent. All
children who attend Head Start receive screenings that include: screening of your
child's ears, eyes, readiness skills, social
skills, large
motor skills and fine motor skills.

Nutritional
Assessment - All parents are asked to complete this form so that the Head Start
Nutrition Consultant can review the assessment.

Health History - An important form that is completed
by parents to provide information and/or update any additional information.
PARENT
ORIENTATION:
Parents
will receive a letter inviting you to a Parent Orientation and for your
child to have the screenings we have talked about conducted.
INITIAL HOME
VISIT:
If
the child the is scheduled to begin school in Mid-September, normally the parent will
receive a home visit from the child's teacher in September. If the child begins later in the year, this visit will take place and include all
of the other information on the June visit. Parents will also be notified
by one of the Family and Community Partnership staff to attend a parent
orientation.
