INITIAL HOME VISIT


If the child is scheduled to begin school in Mid-September, normally the parent or guardian  will receive a home visit from the child's teacher in September.   Parents/guardians will also be notified by a member of our staff to attend a parent orientation.  

 

The following are discussed on the Initial Home Visit:

  • Review of Information Waiver - To make sure that there are no changes.

  • Family Partnership Information - The teacher asks questions and writes  the responses that the parent or guardian gives.  It is a way of assessing family needs, and helps Family Partnership Staff to serve the families throughout the year. Both the teacher and parents sign the form once it is completed.   Head Start wants to ensure that  the child, the parent, and family have an effective year as partners in this educational development.  

  • Behavioral Screening - Parent rates their child's behavior.

  • Consent for Release of Information - Any time a child is receiving additional services, parents are asked to complete this form so that Head Start can work effectively with your child. 

  • Emergency Care - Parents are required to complete this form and sign their signature. 

  • Excused Absences - Parent must complete the excused absence form when their child is absent from school. (Exception: if the child is absent due to a contagious illness, a doctor's excuse is required).

  • Sizes for Holiday Gifts - Each year Voluntary Action and other businesses or organizations donate gifts to all children.  We ask that the parent submit their children's clothing sizes.

  • Agency Service Directory - Contains a listing of all the agency phone numbers in the county.

  • Bus Rules - Will be reviewed again with parent.  

  • Bus Tags and Time for Pick-Up and Return -  Parent will be given a bus tag for their child to wear and the estimated times that the  bus will pick their child up in the morning and drop them off in the afternoon.
